Description ::
Erectra 100mg. an oral therapy for erectile dysfunction (ED) is a citrate
Salt of sildenafil a selective inhibitor of cyclic guannosine monophosphate
(cGMP)-specified,phophodiesterase type 5(PDE5). Sildenafil citrate is
Designed chemically as 1-](3-(6,7 dihydro-1methy1-7-oxo-3-propl-1-H-
Pyrazolo(4,3-d]pyrimidin-5-yl)-4-ethoxy phenyl] sulfony-4-methylpip
erazine citrate and has a molecular weigth of 666.7
Clinical Pharmacology ::
Machanism of action : the physiologic mechanism of erection of the penis
Involves release of nitric oxide (NO) in the corpus cavemosum during
Sexual stimulation. Nitric Oxide (NO) than activates the enzyme guanylate
cyclase, which results in increased levels of cyclic guanosine monophosphate
(cGMP), producing smooth muscle relazation in the courpus cavernosum
and allowing inflow of blood. Sildenafil has no direct relaxant effect on
isolated human corpus cavernosum, but enhances the effect of nitric oxide
(No) by ingibiting phosphodiesterase type 5(PDP5), which is responsible
For degeneration of cGMP in the Corpus cavernosum.
When sexual stimulation casue local release of nitric oxide (NO), inhibition
of PDE5 by sidenafil cause local increased levels of cGMP in the corpus
cavernolsum, resulting in smooth muscle relaxation and inflow of blood
to the corpus cavernosum, Sildenafil at recommended doses has no effect
in the absence of suxual stimulation.
Pharmacokinetics and Metabolism ::
Erectra 100 is rapidly absorbed after oral administration with absolute
Bioavailability with absolute bio-availablity of about 40%. Its pharma-
cokinetics are does proportional over the recommended dose range.
It is eliminated predominantly by heptic metabolism and is converted
to an active metabolic with properties similar to the parent, sildenafil.
Bot Sildenafil & the metabolic have terminal half lives about 4 hours.
Maximum observed plasma Concentration are reached within 30 to
120 minutes (median 60 minutes) of oral dosing in the taste state.
When ERECTRA-100mg. is taken with a high fat meal, the rate of
Absorption is reduced, with a mean delay in Timax of 60 minutes and
A mean recuction in Cmax of 29% . The mean steady state volumed of
Distribution (Vss) for sildenafil is 105L, indicating distribution into the
Tissue. Sildenafil and its major circulating N-desmethyal metabolic are
both approximately 96% bound to plasma proteins.
Contraindication ::
Consistent with its known effects on the nitric exide/cGMP pathway,
ERECTRA 100mg. was shown to protentiate the hypotensive effects
of nitrates and its administration to patients who are using oraganics
nitrates, either regularly and/or intermittently, in any form is therefore
contraindicated. ERECTRA 100 is contraindicated in patients with
a known hypersensitivity to any component of the status.
1. Patients who have suffered a myocardial infarction, stroke or
Life-threatening arrhythmia within the last 6 months.
2. Patients with resting hypotension (BP<90/50) or hypertention
(BP<170/110)
3. Patients with cardiac failure or coronary artery disease causing
Unstable angina.
4. Patients with retinitis pigmented.
Drug Intraction ::
A reduction in sildenafil clearance occurs when it was co-administered
with CYP-3A4 inhibitors such as ketoconazole, intraconazole, erythro
mycin or cimetidine. It can be expected that concomitant admistration
of CYP 3A4 inducers, such as rifampin will decrease the plasma livels
of sildenafil.
Co-administration with HIV protease inhibitors ritonavir/saquinavir
Results in increase in sildenafil Cmax & Plama AUC.
Careinogenesis, Mutagenesis, Imairment of Fertility. Sildenafil did not
Impair the fertility & was not carcinogenic & mutagenic in animal tests.
Pregnancy, Nursing mothers and Pediatric Use
Eretra 100 not indicated for use in new borns, Children or women.
Adverse Reations.
Erectra 100mg. is generally will toerated. Commonly reported adverse
Events of patients treated with sildenafil include the following::
Headache (16%), flusing (10%) dyspepsia (7%), nasal congestion (4%),
Abnormal vision (3%), dizziness (2%), & rash (2%).
Indications ::
Erectra 100 is indicated for the treatment of Erectile Dysfunction (ED)
Adverse Effects ::
In studies with healthy volunteers of single doses upto 800mg. adverse
Events were similar to those seen at lower does but incidence rates were
Increased, In case of overdose, standard supportive measures shoud be
Adopted as required. Renal dialysis is not expected to accelerate clearance
As sildenafil is highly bound to plasma and it is not eliminated and
It is not eliminated in the urine.
Dose and Administration ::
For most patients, the recommended dose is 50mg. taken, as needed,
Approximately 1 hour before sexual activity. However Erectra 100mg.
May be taken anywhere from 4 hours to 30 minutes before sexual activity.
Based on effecyivencess and toeration, the dose may be increased to
Maximum recommended dose of 100mg. or decreased to 25mg.
The maximum recommended dosing frequency is once per day.
The following factors are asspcoated with increased plasma levels
Of sildenafil :age>65 hepatic impairment,severe renal impairment
And concomitant use of potent cytochrome P450 3a4 inhibitors
(ketoconazle, itraconazole, erythromycin, ritonavir and saquinavir).
Since higher plasma levels may increase both the efficacy and
Incidence of adverse events, a starting dose of 25mg. should be considered
In these patients.
